Care Packages
Assessing your Needs
Taking the step of getting support in your home is a big step, but you will reap the benefits of the extra care support.

One of our experienced assessors will visit you to discuss your exact requirements and advise you accordingly. The service is absolutely free and places the enquirer under no obligation. A care plan will be designed to meet your specific needs and a carer will then be assigned to carry out your specific requirements.

Your assessment will outline any adaptions needed to ensure your safety and maintain your independence. The adaptions could include things such as handrails, walk in shower or a stair lift.

Our care team use a digital system called Access Care Planning to document all your notes. These will be reviewed regularly by care managers to ensure you are receiving the best possible care. A paper copy is also kept in your home which include:

-
Your person-centred care plan, health & risk assessment, medication risk assessment, moving and handling assessment, Environmental risk assessment, tissue / skin viability risk assessment and external risk assessments.
-
All details of your next of kin will be stored in your file also.
-
A MAR chart is stored electronically (this is where we document your medication intake). Medication is usually stored in a locked box.
If you require specialist equipment such as a hoist you will need a space for these to be stored away when not in use. If you need assistance with getting the right equipment, we can help with this. Although we do not supply the equipment however, we can place a referral on your behalf to an OT / physiotherapist or district nurse. This does also include continence referral.
How Much Does Care Cost?
There are a number of different funding options available for domiciliary care; your local council may offer financial support, or you may have to fund it yourself. Regardless of your circumstances, please contact us here at Handle With Care and we will help you decide which is the best option for you and your set of circumstances.

A common misconception is receiving a personalised care service at home is beyond the financial means of most people. However, as more information about financing care becomes available, care at home is becoming an increasingly popular option.
Care Funded by Local Authorities
Your local council can arrange home care for you if you’re eligible. In order to determine if you qualify for support from your local authority, a ‘needs assessment’ will need to be completed to determine the type and amount of support required. If the assessment deems you qualify for state-funding, the local council can then organise funding and a care plan and an agency to provide your care will be selected. In the case of a personal budget or direct payment, a sum of money will be offered to cover the cost of care, after which a care provider of your choosing can be introduced. This gives you the control and flexibility to choose the most suitable package and care agency.
Handle With Care is ready to help you with this process and, together with your local council, will determine the best outcome for you and your family.

Personal budget
A personal budget confirms the overall cost of your care package and support the local authority coordinates for you. It specifies what contribution, if any, is required towards the overall cost of care and the balance to be paid by the local authority.

Direct payment
Direct payments are a funding choice in personal budgets and allow for greater flexibility, involvement and control over how your care needs are met. This is achieved by allowing you to decide upon your preferred care and support services. Depending on the type of support that is required, and the amount paid to you as direct payment, you may need to contribute to some of your care costs yourself. To find out if you are eligible for direct payments, please contact your local authority.
Cost of Privately Funded Care
If you are not eligible for state-funding, or want to supplement a care package currently being received, you can finance this as a private customer. Handle With Care are always available to make this process as clear for you as possible.
We appreciate that when deciding to pay for your care or the care of a loved one, you naturally want the very best but finances are a critical consideration and it may not be immediately clear which option is best suited to you. Depending on your budget, there are a number of options available, which we are happy to discuss with you at length. Please contact us by telephone or by email.
